D: depends on what scanner and how thick the book is I have a scanner with a lid so with a doujinshi I simple open it and press it down on the glass, make sure the edges are within the boundaries and put the lid back up. Though this way you can easily mess up and have folds in the middle which can cause the text bubbles there to become blurred or nor scanned, for those pages I just do lid off and hold the book down on the glass, don't press too hard ofc =)
Afterward I use photoshop to re size and adjust the curves to get back the black ink look and remove any extra light that might have come in while scanning
