It is all done by the approval of the studio, like Sakura said those designs were the first Suzaku and Lelouch designs. Apperantly they weren't happy and wanted it more bishie Having someone do a character design doesn't always mean that they are entirely free to design as they like, usually the studio is impressed with their style of drawing and previous designs and they give guidelines to what they want, if the studio doesn't like a first design they will say what they like and then Clamp makes something in response to that The studio wanted them to probably look more like the other characters If this anime was a completely clamp production things would've been different
