But it was shown in the flashbacks all these kids with the signia on their forehead. And she didn't have that scar before she got the code, and you can't get scarred after you have immortality (so BIG CHANCE she got it from the nun).
And her wish, was to die. Her original wish was to be loved, I don't know if it changes, but she said it herself, her wish was to die. Maybe, the revelation of Lelouch's devotion adds to her memory wipe.
